Here are just a few changes that have been used by some of our customers to personalize our standard Application for Employment form. And this can be done with any of our forms - modify, revise, eliminate - to suite YOUR NEEDS! Tell us what you want, send or fax a marked up sample or sketch - specify the quantity and we'll call back with a price.

Replace standard questions that now appear on the stock form with specific questions of particular importance to your company. Here are some examples:

"Do you have a drivers license?", "Do you own a car?", "Will you travel?" or similar, bonafide, occupational qualifications (legitimate job-related criteria). If you wish to check for drugs, add a "Consent to Non-invasive Drug Screening" statement.  
State conditions of employment such as a medical examination, fingerprinting, drug testing.
"Do you have any relatives working for this company?"
"May we contact your former employers?"
"Are there any former employers you do not wish us to contact?"
"Have you ever been employed by us?"
"Have you had computer experience?"
"Please list the job related computer software you are familiar with."
"Approximately how many words per minute are you capable of at a computer keyboard?"
"Have you had any telephone marketing experience?"
"Have you had any sales experience in counter, telephone or outside sales?"
"Have you ever worked in sales on a commission basis?"
 

Provided the question is legitimately job-related, you can obtain specific information that will help you screen potential employees. In the event you use a question that is borderline, our legal department will examine it and we'll advise you accordingly.

OR - customize the "Applicant's Statement" on the back of our Application for Employment to incorporate specific terms of employment, agreements, policy statements, drug screening (if any), investigation and use of credit, medical and personal history, termination provisions, etc.

Use your Company Name in the text of the "Applicant's Statement" instead of the generic "company" as it appears in a stock form.
